I've searched for something like this, but could not find it all over the Internet, so I designed my own. Later I replaced it with a different circuit, much better for my purpose, but the project seemd quite necesary since I'v sold some units on ebay and I still get new asks for it. So, if You want to buy a piece or make it by Yourself - here is the way.
Two pins for power, two inputs and typical 3-pin servo socket.
Three potentiometers to adjust 3 servo states: neutral, pin#1 high and pin#2 high.
Oscillogram shows, it keeps the params quite OK.

I recommend trace it all as top layer and mount all components as SMD, so You get clear bottom surface ideal to mount anywhere with 2-side sticky foam or tape. My versions are made this way.
Application:
As far as I know, some people applied it to control CCTV zoom and/or camera position.
